Fields in the Identification Tab in the Name Screen

The Identification tab in the Adding New Name and Editing Name screens contains the following fields, which are separated by area.

Name area

Use the Name area to track basic information about the name of the individual.
  • Identity Status area: Select a status for the Name record from the following options:
    — Verified: The name information is supported with appropriate documentation.
    — Not Verified: The name information is not supported with appropriate documentation.
    — Partial Name: Only the first or last name of the individual is known.
    — No Name Provided: Only the physical traits of the individual are known. If this option is selected, then the Last Name field changes to the Name field, which is populated with the value No Name Provided, and the First NameMiddle Name, and Suffix fields are removed.
  • Last Name: Enter the last name of the individual. By default, a last name is required. However, if the Partial Name option is selected in the Identity Status area, then the last name is not required.
  • First Name: Enter the first name of the individual.
  • Middle Name: Enter the middle name of the individual.
  • Suffix: Enter the suffix of the name. For example, Junior.

Nicknames/Monikers area

Use the Nickname/Monikers area to track information about other words used to identify the individual.
  • Type: Select the type of other name.
  • Known As: Enter the additional name for the individual.

Addresses area

Use the Addresses area to track addresses associated with the individual.
  • Address Type: Select the type of address. For example, home or work. When viewing the address on the Name record, including when the Name record is displayed as a card on other records, the address type is displayed with the address.
  • Primary check box: If the address is the primary residence of the individual, then select this check box.
  • Address: Enter the street address. See the article "Entering Addresses."
  • Address Directions/Notes: Enter any notes about the address. For example, directions or dangerous animals. This information is connected to the address, apart from the Name record.
  • Start Date: Enter or select the date the individual began living or working at the address.
  • End Date: Enter or select the date the individual stopped living or working at the address. To enter information in this field, the Expired check box must be selected.
  • Expired check box: If the individual is no longer connected to the address, then select this check box.
  • Name Location Notes: Enter any information about how the individual is connected to the address. This information is connected to the Name record, not the address.

Phone Numbers area

Use the Phone Numbers area to track phone numbers associated with the individual. See the article "Entering Phone Numbers."

Former/False/Other Identities area

Use the Former/False/Other Identities area to track information about other identities the individual uses.

To add a stolen identity for an individual, select Stolen Identity, and then locate the record to link to the Name record. See the article "Connecting Records." Complete the following fields.

To add other types of additional identity, select the type of identity, and then complete the following fields:
  • Last Name: Enter the last name of the identity.
  • First Name: Enter the first name of the identity.
  • Middle Name: Enter the middle name of the identity.
  • Suffix: Enter the suffix of the identity.
  • Comments: Enter any comments about the identity.
  • Addresses: If applicable, enter any addresses associated with the identity. See the article "Entering Addresses."
  • Phone Numbers: If applicable, enter any phone numbers associated with the identity.  See the article "Entering Phone Numbers." 

Driver License area

Use the Driver License area to track information about the driver license of the individual.
  • Number: Enter the driver license number of the individual.
  • State: Select the state that issued the driver license.
  • Expires: Enter or select the date that the driver license expires.
  • Classification: Select the classification of the driver license. Multiple classifications can be selected.

Local ID Numbers area

Use the Local ID Numbers area to track local identification numbers for the individual.
  • Type: Select the type of identification number. Your administrator creates the types.
  • Number: Enter the identification number.

Physical Description area

Use the Physical Description area to track the physical characteristics of the individual.
  • DOB: Enter or select the date of birth of the individual.
  • Approximate check box: If the date of birth is unknown, then select this check box. The DOB field is changed to the Age field. Enter an age range for the individual.
  • Race: Select the race of the individual.
  • Ethnicity: Select the ethnicity of the individual.
  • Sex: Select the sex of the individual. If the sex is unknown, then select Unknown.
  • Height: Enter the height of the individual, in feet and inches.
  • (Height) Range check box: If the exact height of the individual is unknown, then select this check box. The Height field is divided into two fields. Enter the range of the height, in feet and inches.
  • Weight: Enter the weight of the individual, in pounds.
  • (Weight) Range check box: If the exact weight of the individual is unknown, then select this check box. The Weight field is divided into two fields. Enter the range of the weight, in pounds.
  • Eye Color: Select the eye color of the individual.
  • Glasses: Select the type of glasses the individual wears.
  • Hair Color: Select the hair color of the individual.
  • Hair Type: Select the hair type of the individual.
  • Hair Style: Select the hair style of the individual.
  • Hair Condition: Select the condition of the hair of the individual.
  • Teeth Condition: Select the condition of the teeth of the individual.
  • Handedness: Select the dominant hand of the individual.
  • Build: Select the build of the individual.
  • Blood Type: Select the blood type of the individual.
  • Facial Hair: Select the type of facial hair of the individual.
  • Complexion: Select the complexion of the individual.
  • Speech Type: Select the type of speech used by the individual.

Identification area

Use the Identification area to track identification numbers of the individual.
  • SSN: Enter the Social Security number of the individual.
  • State ID: Enter the state identification number of the individual.
  • FBI Number: Enter the FBI identification number of the individual.
  • NCIC Fingerprint Classification: Enter the NCIC fingerprint classification number of the individual.

Alerts area

Use the Alerts area to track information about alerts for the individual.
  • Alert: Select the type of alert. Your administrator creates the types.
  • Level: Select the color for the alert. The higher the color is on the list, the higher the priority of the alert using that color. Your agency determines what the colors mean.

Email Address area

Use the Email Address area to track email addresses of the individual.
  • Type: Select the type of email address.
  • Email Address: Enter the email address of the individual.

Scars/Marks/Tattoos area

Use the Scars/Marks/Tattoos area to track identifying features of the individual, including scars and tattoos.
  • Type: Select the type of identifying feature.
  • Scar/Mark/Tattoo: Select the specifics of the mark. For example, the location of the scar, or the drug the individual abuses.
  • Comment: Enter any comments about the identifying feature.
